Intended purpose

The assay attomol® Fibrinogen gamma Realtime TM 2 is used for the supportive diagnosis of thrombophilia. This test can be used to determine the transition 10034C>T in the human fibrinogen gamma gene (FGG gene, rs2066865). It is a manual, qualitative assay using an allel-specific PCR with integrated probe hybridisation. The evaluation takes place by agarose gel electrophoresis, which can be carried out manually or automatically. The sample material has to be genomic DNA prepared from EDTA or citrate blood.

Field of application

Fibrinogen is a coagulation factor consisting of three polypeptide chains (alpha, beta, gamma), which is converted to fibrin by thrombin. Fibrin itself is essential for the appearance of a coagulation thrombus. Abnormalities in this system can lead to an increased risk of thrombosis [Uitte de Willige et al., 2005, Blood, 106: 4176-83]. Grünbacher et al. [Grünbacher et al., 2007, Thrombosis Research, 121: 33-36] were able to show that the mutation 10034C>T (rs2066865) in the fibrinogen gamma gene is associated with such an enhancement. The mutation preferentially produces a splicing variant of fibrinogen gamma, whose protein hasa lower affinity to thrombin [El-Galaly et. al, 2013, British Journal of Haematology, 160:838-41]. The reduced affinity causes the fibrinogen concentration in the plasma to rise [Uitte de Willige et al., 2005, Blood, 106: 4176-83],  which  is  associated  with  increased  plasma  viscosity  and  platelet  aggregation.  The  incidence  of  a homozygous mutated genotype for polymorphism 10034C>T is about 5.4 % and around 35.2 % for the heterozygous genotype [Grünbacher et al., 2007, Thrombosis Research, 121: 33-36].
